The University of Oklahoma Health Sciences Center OneCard Office is responsible for the production and printing of the identification cards used by students, faculty and staff on the Health Sciences Center campus.

The cost of an initial identification card for all students is $15.00.  A replacement ID card is issued at a cost of $20 for students and faculty/staff.  Generally, the cost of replacement cards is the responsibility of the cardholder. 

In the event a student identification number must be changed due to an administrative directive, a new ID card must be issued.  Departments or colleges whose actions result in changes to student identification numbers are responsible for the cost of replacement ID cards. The OneCard Office will contact the responsible college/department to obtain a chartfield spread to charge the cost of the new student ID cards.
